"validation_description": "
We identify anxiety with Read Codes in primary care using a collection of concepts in combination.
\r\n
\r\n
A full algorithm has been developed and validated and includes codes denoting anxiety diagnosis and symptoms, as well as prescriptions of anxiolytics and hypnotics that coappear with a previous diagnosis of anxiety. Referrals for therapeutic procedures that unequivocally denote a diagnosis of anxiety are also included. Code lists for each of these concepts are based on validated published and comparable lists [John et. el. 2016; Cornish et al 2016; John et. al., 2022] and reviewed by a clinician in the study team (Prof Ann John). The algorithm was originally developed and validated using the MHI-5 as the gold standard [John et al., 2016].
\r\n
\r\n
The algorithm can be applied using multiple combinations of concepts tailored to the study question. As a minimum, the algorithm should include the core anxiety diagnoses and symptoms. Anxiety disorders of childhood, Mixed Depression and Anxiety, prescribing codes and referral for therapeutic procedure codes may also be included and used by the algorithm. Which definition and combination of concepts to use depend on the study question and must be discussed with an expert clinician.
\r\n
\r\n
<a href=\" /”target =\"_blank\"> Anxiety Diagnoses (core)
\r\n
\r\n
These codes refer to the common mental disorders (CMD) diagnosis of anxiety. This definition encompasses panic attacks and panic disorders (John et al., 2015), codes for phobias (Cornish et al., 2016) and obsessive compulsive disorders (John et al 2019; John et al., 2022) and PTSD (Moran et al., 2015) These codes should be used in combination with anxiety symptoms (below) as a minimum to identify anxiety in primary care.
\r\n
\r\n
Mixed Depression and Anxiety and anxiety disorders with onset in childhood are distinct from this list and are included as separate concepts below to be included where appropriate
\r\n
\r\n
Anxiety Symptoms (core)
\r\n
\r\n
These codes refer to symptoms of anxiety. As a minimum these codes should be included with the core diagnoses codes when identifying anxiety.
\r\n
\r\n
Clinicians are increasing using symptom rather than diagnoses codes to record anxiety in primary care in both children and young people (John et al., 2015) and adults (Walters et al., 2012). As such it is important that these codes are included when identifying anxiety in primary care.
\r\n
\r\n
Anxiety diagnosis – diagnosis specific to childhood
\r\n
\r\n
This list can be used in conjunction with anxiety diagnosis, anxiety symptoms, mixed anxiety and depression treatment and prescribing codes to identify anxiety in children and young people in primary care
\r\n
\r\n
Mixed Depression and Anxiety
\r\n
\r\n
These codes refer to a clinician record of mixed depression and anxiety. They can be used in conjunction with diagnostic or symptom codes to identify depression or anxiety. However, if a coding hierarchy is to be employed based on that used by clinicians, mixed depression and anxiety would be preferentially allocated to identify depression
\r\n
\r\n
Anxiety – Therapeutic Procedures
\r\n
\r\n
This list refers to therapy and referrals for anxiety. \r\n.
\r\n
\r\n
\r\n
Anxiety Prescriptions
\r\n
\r\n
These codes refer to prescriptions of anxiolytics and hypnotics. They can be used to identify prescriptions of anxiolytics and hypnotics independent of diagnosis. They can also form part of the algorithm for identifying anxiety in primary care when an individual has a previous diagnosis of anxiety recorded.
\r\n
Anxiolytics
\r\n
\r\n
Prescription codes for anxiolytics.
\r\n
\r\n
Hypnotics
\r\n
\r\n
Prescription codes for hypnotics.
",
