The most recent smoking record before study entry was used to classify individuals as never, ex- or current smokers, and those identified as current smokers with no smoking record within the 3 years before study entry were reclassified as having missing smoking data. Never smokers who had a record of smoking at any time before baseline were reclassified as ex-smokers.
